The Columbia University Student Health Insurance Plan
(the Columbia Plan) covers a broad range of healthcare needs
and complements services available on campus through
Columbia Health.
The Columbia Plan (Aetna Open Choice PPO Plan) helps to
manage the cost of medical and mental health services that
are not provided on campus, such as hospitalization, x-rays,
prescriptions, specialist visits, etc.
1. Print your Insurance ID from aetnastudenthealth.com/columbia
using your UNI and date of birth.
2. Make a primary care appointment with Medical Services or
treatment planning session with Counseling and Psychological
Services before seeking off-campus care.
3. Have a referral from Columbia Health? Find an off-campus
provider with Aetna DocFind at aetnastudenthealth.com/
columbia.

PLAN HIGHLIGHTS
1. $0 deductible. You don’t have to spend a dollar
on healthcare expenses before coverage kicks in.
2. $15 generic, $50 preferred brand, $75 non-preferred
brand prescription co-pays. There is no co-pay for generic
contraception.
3. Co-pay for in-network, off-campus visits with a referral from
your Columbia Health provider: $30 for medical visits; $0
for the rst 10 mental health visits, $20 for subsequent visits.
There is no co-pay for on-campus medical and mental
health visits.
4. $30 co-pay for in-network services such as lab tests,
x-rays, etc.
5. $60 Urgent Care visits, $150 ER visits.
6. No referral required if the provider is more than 50 miles
from campus.

HOW TO USE THE COLUMBIA PLAN
Primary medical and mental health care:
Make an appointment with Medical Services via
secure.health.columbia.edu or by calling 212-854-7426.
Schedule an initial treatment planning session with Counseling
and Psychological Services (CPS) via secure.health.columbia.edu
or by calling 212-854-2878.
Referrals are mandatory for most off-campus services within 50
miles of campus. If you need to see a specialist or access long-term
mental health care, your provider at Columbia Health will provide
you with a referral. If you receive healthcare services without a
referral, you will be subject to higher fees. Columbia Health cannot
issue referrals after you have received off-campus care.
Note: Referrals are not required for urgent care or the ER, however
please contact Medical Services for all follow-up care from an
urgent care or ER visit to receive a valid referral.
To locate in-network Aetna providers (after you receive a referral),
visit Aetna DocFind at aetnastudenthealth.com/columbia.
For off-campus care, make sure you have your insurance card with
you and expect to pay your off-campus specialist co-pay.
In an emergency:
On campus, call 212-854-5555. For medical emergencies on
or near campus, request CUEMS for free, condential, 24/7
ambulance service.
Off campus, call 911.
For urgent medical concerns, call 212-854-7426.
For urgent mental health concerns, call 212-854-2878.
For sexual assault, call a survivor advocate at 212-854-4357.
All emergency numbers can be reached 24/7.
For the emergency room, Mount Sinai St. Luke’s Hospital on 1111
Amsterdam Ave at 113th St. is the closest medical and mental
health ER to the Morningside campus.
To fill a prescription:
Your Columbia Health provider will electronically send your
prescription to the pharmacy of your choice. Make sure you
have your insurance card or the Aetna Student Health app with
you when you pick up your medication and expect to pay your
pharmacy co-pay.
While traveling outside the U.S.:
In an emergency, please contact On-Call International at
1-603-328-1951 (collect calls accepted from anywhere in the world).

The Columbia Plan and Your Dependents
If you have a spouse or domestic partner as an enrolled
dependent, they must receive their primary care on campus at
Medical Services. They do not require a referral for mental health
services. Dependent children cannot be seen at Columbia Health
regardless of age; they can access off-campus care without a
referral at any time.
Making the most of your Columbia Health resources:
You have access to all the services offered by Columbia Health,
not just Medical Services and CPS. Visit Alice! Health Promotion
for custom programs to help you care for your overall health and
well-being; Disability Services for accommodations and support
services to facilitate equal access to academic programs; and
Sexual Violence Response for outreach, advocacy, and resources
for survivors and co-survivors of sexual violence.
